Initiation Report

We have updated the initiate reporting system; you will use this system to update the status of previously reported associate members. Before you begin the process of updating the status of your associate members, please read all instructions carefully.

When you click the link at the bottom of this page, you will be directed to a list of the current associate members for your chapter. If the name of the individual you wish to update is not listed, please make sure that another officer has not already reported that member as depledged or initiated. If the individual(s) was never reported as an associate member, you will first need to complete an Associate Member Report for that individual(s).

Once you are ready to make updates, be sure that you have the following information available for each new initiate/depledged member:

  • member name
  • roll book number (if applicable/available)
  • initiation date/depledge date

To update the status of an associate member to initiated, locate that member's name in the list, enter the member's roll book number if available, select "Initiate" under status type and select the appropriate initiation date.  To update the status of an associate member to depledged, locate that member's name in the list, select "Depledge" under status type and select the appropriate status change date. If the individual will remain an associate member, leave the status type as "Remain as Associate."

If you would like member badges, cards or certificates for newly initiated members sent to a different address than the pre-populated chapter address, please update this section before submitting the final list.

Once you click the submit button, your list will be uploaded to our database and you will not be able to change any of the member data.

If you need to sign out of your member account and work on your list at a later time, the system will save any updated member information you have already changed.

Once your list is complete, click the "Submit for Approval" button.

Allow 24-48 hours for IHQ to approve your associate member report submission

Once your submission is approved, your chapter president and treasurer will receive an invoice, by email, for the new initiated member fees.  There will be a $20 late fee for each member initiated who is not reported and paid for within 7 days after initiation.The chapter may choose to make the payment online by following the link in the email or by sending in a check.
